Decentralized Clinical Trials (DCTs) refer to a modern study model where trial activities are brought directly to the participant, leveraging digital innovations, local healthcare facilities, and home-based visits to reduce or eliminate the need for traditional, in-clinic center visits.

While the COVID-19 pandemic catalyzed rapid, necessity-driven interest in DCTs to bypass global lockdowns, trials from ClinicalTrials.gov reveals an industry that is still meticulously constructing its methodological foundation. Far from being a fully matured "new normal," the current DCT landscape remains largely in a developmental phase, with majority of ongoing trials focused on evaluating and validating decentralized methodologies themselves rather than merely executing them. The data underscores a highly heterogeneous ecosystem where behavioral interventions currently demonstrate the highest suitability for remote execution compared to other treatment modalities. In this framework, digital technologies play a dual role: they serve both as the core clinical interventions being tested and as the critical operational infrastructure enabling practical implementation steps such as remote trial delivery, online screening, and real-time patient monitoring via wearable devices.

However, moving from theoretical concept to global implementation reveals significant geographic disparities, as digitalized DCTs remain heavily concentrated within high-income nations, even though over 42% of the analyzed trials involve sites or partners outside the United States. While the theoretical promise of DCTs is vast offering the potential to dismantle geographic barriers, optimize data collection costs, shorten drug development timelines, and enhance patient diversity the clinical research enterprise must bridge substantial operational gaps to achieve widespread, equitable adoption. To fully unlock the paradigm-shifting potential of remote trials, particularly for complex therapeutic areas like rare diseases or populations with severely limited mobility, the global research community must strategically prioritize four critical pillars: advancing digital equity across diverse patient demographics, establishing harmonized regulatory frameworks for rapidly evolving technologies, implementing standardized methodological validation processes, and conducting rigorous research into localized implementation barriers.
